Healthcare RAG System

Privacy-First Clinical Decision Support Architecture

A privacy-first, locally-hosted retrieval-augmented generation (RAG) system designed for clinical decision support in HIPAA-regulated healthcare environments. The architecture targets high-value use cases including prior authorization review, care gap identification, and patient engagement content generation - enabling healthcare organizations to adopt AI capabilities without transmitting sensitive patient data to external providers.

Built on a hybrid retrieval approach combining BM25 keyword search with semantic vector search, the system improves accuracy on clinical terminology where the same condition may be described in multiple ways across different guide lines and documentation. All inference runs locally on-premise - no PHI leaves the organization's infrastructure.

Clinical Use Cases

  • ✓ Prior authorization review against payer guidelines
  • ✓ Care gap identification and outreach support
  • ✓ Patient engagement content generation
  • ✓ Regulatory document analysis within HIPAA-compliant infrastructure
  • ✓ Air-gapped deployment for maximum data sovereignty

Architecture Highlights

  • pgvector - Vector database layer for semantic similarity search
  • Hybrid Retrieval - BM 25 + semantic search with RRF fusion for clinical terminology accuracy
  • Local LLM (Ollama) - On-premise inference, no external API calls
  • Docker - Containerized, portable, reproducible deployment
  • HIPAA-Compliant by Design - No patient data transmitted externally

Why This Matters

Healthcare organizations want AI capabilities but face a fundamental constraint: patient data cannot be sent to hosted AI providers under HIPAA. This architecture solves that problem directly - delivering generative AI functionality entirely within the organization's own infrastructure. The hybrid retrieval approach addresses a real clinical challenge: medical terminology is inconsistent across guidelines, payer documents, and clinical notes. Combining keyword and semantic search produces significantly more accurate retrieval than either method alone.
